Photo ID Sheets 2022/23 Season

Every player MUST be registered on the Whole Game System (this is an FA requirement). We, as a league, also require all players to be signed on using the league’s photo ID sheet. This means managers must present two passport photos on the sheet attached for each child, with the details also completed. We’ve included an example. Please ensure this is completed and returned to us on August 28th at Caldway Drive, to ensure that the players are signed on in time for the first league match.
